Physical Therapist (PT) - Acute Care

About the Role

Evaluates and treats patients of all ages, developing and modifying personalized care plans based on patient response and evidence. Acts as a professional role model and clinical resource for staff and students.

Qualifications

• Bachelor's Degree in Physical Therapy
• Licensed as a Physical Therapist (PT) in Illinois
• BLS certification within 30 days of hire
• Must complete ongoing education related to acute stroke care

Responsibilities

• Perform patient transfers safely, using proper body mechanics
• Provide respectful, empathetic care, maintaining patient dignity
• Design, adjust, and implement evidence-based treatment plans
• Educate patients, families, and healthcare team members effectively
• Evaluate clinical findings, perform examinations, and document in compliance with policies
• Screen patients for therapy needs and monitor support personnel's care delivery
• Utilize functional activities and standardized outcome measures
• Collaborate with patients and families, provide education, and supervise support staff
• Communicate patient progress, discharge plans, and equipment needs clearly to the medical team
