Your Role

As a Physical Therapist, you will provide professional, evidence-based care in accordance with physician orders and agency policies. Help clients regain independence and improve quality of life through personalized rehabilitation plans.

Key Responsibilities

• Conduct comprehensive assessments and develop individualized treatment plans
• Deliver therapeutic interventions (manual therapy, modalities, exercise programs)
• Educate clients, families, and caregivers on therapy techniques and home programs
• Supervise PTAs and Home Health Aides (as permitted)
• Collaborate with interdisciplinary teams and participate in care conferences
• Monitor progress and adjust treatment plans as needed
• Maintain accurate, timely documentation

Qualifications

• Degree from an APTA-accredited Physical Therapy program
• Active state PT license
• 1+ year of experience or completion of a 6–8 week preceptorship
• Valid driver's license and reliable transportation
• Strong organizational and time-management skills

Physical & Mental Requirements

• Ability to lift up to 35 lbs and assist with mobility
• Frequent standing, walking, bending, and positioning
• Adaptability to changing schedules and environments
• Travel to client homes and Trinity Health locations required
