Physical Therapist - PRN

Role Overview Seeking a PRN Physical Therapist (PT) to evaluate and treat residents, enhancing mobility, strength, and independence. Collaborate with a multidisciplinary team to deliver personalized, high-quality care. Key Responsibilities Conduct comprehensive assessments of residents' physical abilities.Develop and implement tailored treatment plans to improve functional outcomes.Provide therapeutic interventions such as exercises, manual therapy, and assistive device training.Monitor progress and modify therapy plans accordingly.Educate residents and families on therapy goals and exercises.Accurately document evaluations, treatments, and progress notes per standards.Participate in quality improvement initiatives. Qualifications & Skills Doctorate or Bachelor’s/Master’s in Physical Therapy from an accredited program.Valid Physical Therapist license in Colorado.Experience in skilled nursing or rehab settings preferred.Strong communication, teamwork, and resident-centered approach.Ability to work independently with flexible scheduling.Compassionate and dedicated to effective therapy services.
