Physical Therapist PRN

We are seeking a PRN Physical Therapist (PT) to provide home health services in Orange City, Florida. Responsibilities Include Evaluating patients' functional status, mobility, endurance, pain, and medical conditions.Developing and implementing individualized treatment plans in collaboration with physicians and the healthcare team.Performing interventions to improve patient well-being and restore or maintain function.Monitoring and reporting patient progress and responses to treatment.Educating patients, families, and caregivers about the care plan and functional goals.Completing patient evaluations, progress notes, and reports on outcomes and care frequency. Qualifications Include Current PT licensure in the state of practice.CPR certification.Valid driver’s license, vehicle insurance, or access to public transportation.Ability to work effectively in diverse home environments and with individuals of all backgrounds.
