Physical Therapist PRN

About the Role

Folkston Park Care & Rehab is seeking a Per Diem (PRN) Physical Therapist to join our in-house team of professionals in Folkston, GA. We provide person-centered care to achieve positive outcomes for our residents.

Responsibilities

• Consult with patients to learn about their physical condition.
• Assess and interpret patient evaluations and test results.
• Develop treatment plans using a variety of treatment techniques.
• Create personalized rehabilitation programs for patients.
• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely, complying with all company and state/federal regulatory requirements.
• Comply with all rules, regulations, and procedures.
• Attend required meetings as designated by the Director of Rehab.

Requirements

• Active/Valid GA license in Physical Therapy.
• In good standing with all regulatory agencies and licensing boards.

Skills

• SNF/Long-term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.
