About the Role

The Physical Therapist in Home Health assesses patients' functional status, mobility, endurance, and pain to develop personalized treatment plans. The role requires adapting to various home environments, ensuring quality care regardless of patient background, and delivering equitable, high-quality care in diverse settings.

Responsibilities

• Provide therapy within legal scope of practice.
• Monitor patient responses to treatment and adjust plans as needed.
• Instruct care team members and families on patient care techniques.
• Document patient progress and outcomes.
• Complete evaluations and develop plans of care per state guidelines.
• Report outcomes and ensure compliance with regulatory requirements.

Requirements

• Current state licensure in Physical Therapy.
• Valid driver’s license, vehicle insurance, or access to reliable transportation.

Qualifications

• Preferred: CPR certification or ability to obtain within 90 days.
