About the role
Provide personalized, in-home and senior living care to older adults using evidence-based treatments to enhance mobility, independence, and quality of life.
Responsibilities
- Deliver one-on-one therapy sessions in private residences and senior communities
- Perform evaluations, develop individualized care plans, and implement treatments
- Document in EMR systems and collaborate with interdisciplinary teams
- Educate patients, caregivers, and staff to promote safety and aging in place
- Utilize telehealth and virtual care to extend service accessibility
Qualifications & Skills
- Valid Physical Therapy license or eligibility in the state of practice
- Degree from an accredited Physical Therapy program
- Proficiency in computer use and EMR systems
- Current CPR certification
Physical Requirements
- Ability to perform moderate physical effort
- Assist with patient transfers, repositioning, and therapeutic activities
- Capable of driving and wearing PPE as needed
