Physical Therapist, Per Visit, Homecare, Thompson, CT
Job Summary
Under the supervision of the HomeCare Rehabilitation Supervisor, the Per Visit Physical Therapist organizes and administers prescribed physical therapy programs in an effort to reduce and/or prevent disability and to assist patients in achieving maximum functional potential.
Job Responsibilities
- Evaluates and treats patients as prescribed by referring physician.
- Records treatment, response, progress and discharge information in patient’s electronic chart using portable electronic device.
- Educates patient, family and significant others regarding condition, treatment and home program.
- Routinely confers with physician and other practitioners in person, by phone, or in writing, regarding progress, regression or change of plan.
- Provides clinical supervision of the Physical Therapist Assistant related to the implementation of the patient’s plan of care, in accordance with Medicare Conditions of Participation and state practice regulations.
Education and Experience Requirements
- B.S. Degree and graduate from an accredited school of physical therapy required.
- Certified Connecticut license or license eligible required.
- CPR Certification required.
- Minimum of one year experience preferred.
- Valid Connecticut Driver’s License and proof of current insurance coverage required.
Knowledge, Skills and Abilities
- Proficient in Reasoning, Math, Reading, Writing, and Speaking.
- Data: Determines time, place, and sequence of operations or actions to be taken on the basis of analysis of data, executing determination of and/or reporting on events.
- People: Follows instructions and procedures in fulfilling duties. May attend to the needs, requests of people. Immediate response is usually involved.
- Things: Uses body members, hand tools, special devices to work, move, carry objects or materials. Involves little latitude for judgement with regard to attainment of standards.
- Uses eyes, hands and fingers with skill.
- Communicates with people when they are sick, handicapped or nervous.
- Changes from one duty to another frequently.
- Maintains discretion in handling confidential nature of information available.
- Keeps up to date on trends and developments in the field of physical medicine and health.
