About the role

The Physical Therapist evaluates, plans and implements a goal oriented Physical Therapy treatment program that focuses on restoring function and preventing further disability as a result of trauma, disease, amputation, congenital defects, surgery and other debilitating illness.

Responsibilities

• Participates on committees as requested by Supervisor.
• Delivers the highest level of service to all residents, families and visitors.
• Models and maintains a safe environment in which to deliver services and employs correct body mechanics during activities.
• Demonstrates and implements appropriate treatment techniques and skills with residents.
• Completes resident treatments as scheduled.
• Establishes discharge planning at initial evaluation and updates throughout the course of therapeutic treatment.
• Completes discharge summaries in a timely manner, reflecting goal achievement and functional status with appropriate recommendations.
• Demonstrates appropriate knowledge in assessing the need for Durable Medical Equipment (DME) to improve residents' functional independence and safety.
• Follows through with appropriate professionals to ensure proper fitting of wheelchairs and educates residents/families about proper use and care.
• Prepares written documentation such as Progress Notes, Reassessments, and Discharge Summaries in compliance with departmental policy.
• Completes re-evaluations in a timely manner and updates goals and treatment plans as per departmental policy.
• Develops rapport and mutual respect with residents to foster an optimum environment for goal achievement.
• Provides education to residents/families based on identified learning needs.
• Gathers all necessary information to properly evaluate residents' needs, including testing and consultations with other healthcare professionals.
• Handles special projects and rush assignments as needed.
• Develops a rapport with patients/caregivers and team members to foster goal achievement.
• Provides and documents education to patients/caregivers based on identified learning needs.
• Develops and implements an individualized treatment program based on established goals.
• Completes patient treatments within acceptable departmental productivity range and documents on daily charge sheet.
• Demonstrates appropriate judgment in ordering and measuring residents for prosthetic/orthotic devices.
• Assesses the fit and function of devices and educates residents/families about proper use and signs of malfunction.
• Formulates functionally based, objective short and long-term goals.
• Administers evaluations in a manner appropriate to the resident's disability, demonstrating respect for their needs.
• Provides therapy in a safe environment, utilizing proper body mechanics.
• Develops and implements an individualized program of treatment focused on identified resident needs.
• Participates in the ongoing development of the discharge plan.
• Provides appropriate assistive/adaptive devices and durable medical equipment to improve residents' independence in function.
• Ensures proper utilization and clinical appropriateness of devices/equipment and educates residents/families about their use and safety.
• Adheres to HMH Organizational competencies and standards of behavior.
• Performs other duties and/or projects as assigned.

Requirements

• Graduate of an APTA accredited program or equivalent.
• Physical Therapist License.
• AHA Basic Health Care Life Support HCP Certification.

Skills

• Standing, walking, and/or continuously moving around.
• Kneeling, stooping, crouching, crawling, or working in other unusual physical positions.
• Climbing (including ladders, stepping stools, and/or stairs) or balancing.
• Stretching and/or reaching to retrieve/replace objects.
• Pushing, pulling, lifting, and/or carrying heavy objects (20 lbs. or more) without aid of physical devices.
• Dexterity and eye-hand coordination for activities involving holding, positioning, manipulating, typing, etc.
• Visual ability to monitor instruments and computers, distinguish characters, recognize colors, observe physical characteristics, etc.
• Hearing ability to recognize sounds, sound patterns, and other auditory stimuli.
• Physical ability to react/respond quickly in emergencies, including life-threatening situations.
• Excellent written and verbal communication skills.
• Proficient computer skills, including Microsoft Office and/or Google Suite platforms.

Pay

Minimum rate of $46,072.00 annually. The starting rate of pay is provided for informational purposes only and is not a guarantee of a specific offer. Posted rates may vary based on labor market data, experience, education, certifications, skills, geographic location, internal equity, budget, and grant funding. Some positions may also be eligible for shift differentials, performance-based incentives, bonuses, or commissions.
