Physical Therapist – Outpatient Therapy

We are seeking a Physical Therapist for a full-time, mobile outpatient role providing rehabilitation services to older adults in home and community settings. Key Responsibilities Include Assessing patients, developing and implementing individualized treatment plans to restore and improve function.Educating patients and caregivers to promote independence and health maintenance.Referring patients to other disciplines as needed and collaborating with the rehabilitation team.Supervising support staff, completing documentation, and ensuring safety and proper equipment use.Performing therapeutic techniques such as exercises, manual therapy, modalities, and adaptive device training.Conducting home and community mobility assessments and modifying plans based on patient progress. Qualifications Include Valid Physical Therapy license or eligibility in the practice state(s).Master’s degree in Physical Therapy or related field, or Bachelor’s plus 5 years of experience.Reliable transportation and a valid driver’s license.
