About the role

Adventist Health Ukiah Valley has been deeply connected to the community since 1956. We are comprised of a 50-bed acute care medical center, level IV trauma center, level II intensive care nursery services, extensive physical rehabilitation program, outpatient surgery and diagnostic center and advanced wound care center. Responsibilities

  • Completes evaluations to ensure quality standards and addresses assessment areas that require attention.
  • Uses appropriate clinical techniques and tools and establishes individualized treatment plans for patients.
  • Provides therapy interventions using appropriate treatment protocols and techniques.
  • Establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties.
  • Completes comprehensive and accurate clinical documentation within established deadlines.
  • Supervises assigned assistants.
  • Performs other job-related duties as assigned.

Requirements

  • Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016; or foreign equivalent.
  • Two years' Physical Therapy experience: Preferred.
  • Physical Therapy licensure in state of practice: Required.
  • Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy: Required.
  • Facility Specific License/Certifications: Basic Life Support (BLS) certification from approved vendor per AH policy: Required.

Additional individual compensation may be available for this role through differentials, extra shift incentives, bonuses, etc. Base pay is only a portion of the total rewards package, and a comprehensive benefits program is available for qualifying positions. The estimated base pay for this position is $51.09 to $70.07. Additional individual compensation may be available for this role through differentials, extra shift incentives, bonuses, etc.
