About the Role

Provide rehabilitative care to seniors in skilled nursing and long-term care settings.

Responsibilities

• Implement individualized treatment plans under the supervision of a Physical Therapist.
• Conduct therapeutic exercises, gait and transfer training.
• Educate patients and families about treatment and progress.
• Document patient progress accurately.
• Collaborate with an interdisciplinary team to optimize patient outcomes.
