Physical Therapist or Physical Therapist Assistant

Role: Physical Therapist Assistant supporting high-quality rehab services for seniors in skilled nursing and post-acute settings. Responsibilities Implement physical therapy treatment plans, including therapeutic exercises, gait and balance training, and functional mobility interventions.Document patient progress accurately in EMR systems.Collaborate with an interdisciplinary team to ensure comprehensive patient care.Educate patients and their families to promote safety, independence, and improved quality of life.
