About the role

Join our team as a Physical Therapist at Family Life Care (FLC), where you will provide compassionate and professional care to patients in the comfort of their own homes. FLC serves the entire community, from children to senior citizens. You’ll have the flexibility to choose from various assignments, such as patient case manager, long-term care, or assisting with recovery from an injury or illness. We work with you to build a schedule that meets your needs.

Responsibilities

• Conduct patient assessments, admissions, and coordinate care with other disciplines, physicians, and care managers.
• Provide Physical Therapy and treatment to home care patients in accordance with the plan of treatment and company policies and procedures.
• Deliver Physical Therapy in a pleasant, professional manner to maximize positive outcomes and patient satisfaction.
• Ensure all clinical assessments, care plans, starting, and resumption of care are completed within agency guidelines, with a focus on meeting deadlines.
• Train, coach, and direct clinical staff in the performance of their duties.
• Ensure compliance with all state and federal laws, regulations, policies, and processes relevant to home health.

Requirements

• Current Florida Physical Therapy license.
• Current CPR Certification.
• Valid Driver’s License and Vehicle Insurance.
• Current TB test with negative results.
• Ability to pass a Level II background check.

Qualifications

• Positive, approachable, and mentoring personality.
• Understanding of state and federal laws and regulations related to home health, especially Medicare and Medicaid.
• Experience with Medicare (OASIS), Medicaid, and commercial insurance in the home health environment is desired.
• Strong interpersonal and communication skills, organizational abilities, and attention to detail.
• Comfort with technology, including Electronic Medical Records (EMR) and Electronic Visit Verification (EVV) systems.
• A "go the extra mile" attitude.
