• Evaluation / Re-Admission: $80.00
• Routine Visit: $70.00

Actify Home Care is a clinician‑owned and operated agency led by a Physical Therapist with 20 years of experience. The company emphasizes clinician‑to‑clinician support, a high‑yield per‑visit pay model, and efficient documentation using Kinnser (WellSky). Active home health cases are ready for assignment with rapid credentialing and onboarding.

Responsibilities

• Provide high‑quality, one‑on‑one physical therapy to geriatric patients in their homes.
• Implement effective plans of care to improve patient mobility and safety.
• Complete Evaluations, Re‑Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge) with clinical team support.
• Collaborate with clinical leadership, dedicated office staff, and fellow therapy assistants to deliver outstanding patient care.

Requirements

• Active Florida Physical Therapist (PT) license.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.
• Kinnser/WellSky experience is a major plus.
