- Start of Care (SOC): $110.00
- Evaluation / Re-Admission: $80.00
- Routine Visit: $70.00

Territory
South Broward County: Hollywood, Pembroke Pines, Miramar, Davie, Weston, Hallandale Beach, and surrounding areas.
About the role
- Provide high-quality, one-on-one physical therapy as a field PT to geriatric patients in their homes.
- Implement effective plans of care to improve patient mobility and safety.
- Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge) with a clinical team that helps you get it right.
- Collaborate with our clinical leadership team, dedicated office staff, and fellow therapy assistants to deliver outstanding patient care.
We utilize Kinnser (WellSky) for streamlined charting.
Requirements
- Active Florida Physical Therapist (PT) License.
- Current CPR/BLS Certification.
- Valid Driver’s License and reliable transportation.
- Kinnser/WellSky experience is a major plus.
