We are seeking a Home Health Physical Therapist (PT) traveler for a 13-week assignment in Portland, Oregon. This vibrant city, known for its arts scene, beautiful parks, and relaxed lifestyle, serves as an excellent base for exploring the Pacific Northwest.

Responsibilities

• Provide skilled physical therapy services in a home health setting.
• Develop and implement individualized treatment plans to improve patient mobility, reduce pain, and restore function.
• Utilize Homecare Homebase or similar systems for documentation and care coordination.

Requirements

• Active Physical Therapist (PT) license (Compact accepted).
• Minimum of 1 year of home health experience.
• Valid Social Security Number (SSN) and date of birth (DOB) for compliance.
• One professional reference covering your entire work history.

Preferred Qualifications

• Experience with Homecare Homebase or similar home health software.

Pay

Pay rate: $2,676.00 – $2,758.00 per 8-day shift.
