Physical Therapist Home Health Fulltime
Key Responsibilities
- Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
- Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.
- Collaborate with the interdisciplinary care team to optimize patient outcomes.
- Provide education and support to patients and families/caregivers on therapy programs.
- Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
- Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
- Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
- Identify patient and family/caregiver needs for additional services and make appropriate referrals.
- Confirm daily visit schedules to ensure seamless coordination with other staff.
- Communicate patient progress and changes with physicians and care teams.
- Document all visits and progress notes on the same day of service.
Requirements
- Must have a current PT license (state-specific).
- Graduate of an accredited physical therapy program.
- Valid BLS certification.
- Minimum one year of PT experience (home health experience preferred).
- Excellent communication, organizational, and time management skills.
- Must have a valid driver’s license and access to a reliable and insured vehicle.
- Able to visit homes that may not be wheelchair accessible.
- Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
- Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
- Proficiency with electronic medical records is a plus.

Qualifications
