Physical Therapist (Home Health) – Full-Time

About the role

HCR Home Care is seeking a Full-Time Physical Therapist to provide skilled therapy services in patients' homes in Warren and Washington Counties.

Responsibilities

• Carry out comprehensive evaluations using diagnostic muscle, nerve, joint, and functional ability tests and measures.
• Prepare patient assessments and individualized written plans of care including recommended treatment types and frequency.
• Communicate care recommendations and modifications to physicians as needed.
• Deliver therapeutic interventions aimed at maximizing patients’ functional independence and mobility.
• Document clinical notes and maintain accurate, timely records.
• Educate patients and caregivers on treatment goals and safe use of assistive equipment (e.g., canes, walkers).
• Participate in staff meetings, case conferences, and professional development.
• Ensure safety protocols and proper maintenance of equipment.

Requirements

• Graduate of an accredited Physical Therapy program.
• Current, valid New York State Physical Therapy license.
• At least one year of practical experience as a licensed Physical Therapist.

• Ability to work independently and as part of a team.
• Strong organizational and documentation skills.

• Skilled in conducting physical therapy evaluations and treatments.

Work Environment
