Job Description

In this pivotal role, your expertise will guide one-on-one therapy services to patients in their homes, crafting a significant impact on their recovery and wellbeing. Duties will span from assessing patient needs, sculpting individualized treatment plans, conducting therapy sessions, and vigilantly tracking patient progress.

Key Responsibilities

• Conduct thorough in-home patient evaluations.
• Formulate and enact effective treatment plans, distinctly tailored to each patient’s needs and objectives.
• Educate and continuously engage with patients and their families about home exercise programs and other pertinent care details.

Qualifications

• Current licensure, or eligibility for licensure, as a Physical Therapist in NJ.
• Graduate from an accredited physical therapy program.
