Physical Therapist, Home Health

About the Role

Responsible for assessing, planning, organizing, and participating in rehabilitative programs that improve patient mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury. Demonstrates advanced knowledge and practice level competencies and is a recognized clinical or workflow subject matter expert. Responsible for developing key workflows and clinical protocols or guidelines for the department. Serves as a resource in the education, training, and mentoring of other staff and as a clinical instructor for assigned students. May coordinate staff education and student programs. Adheres to all local, state, and federal regulations, codes, policies, and procedures to ensure privacy and safety while delivering optimal patient care.

Responsibilities

• Collaborates with key stakeholders in developing solutions.
• Applies critical thinking, complex problem-solving, and decisive judgment while working independently.
• Develops and implements treatment protocols.
• Analyzes and identifies clinical practice and workflow best practices.
• Extracts evidence-based practice information and applies it to clinical practice and training materials.
• Develops training materials and competency assessments.
• Teaches, educates, and mentors others, including public speaking engagements.
• Complex problem-solving within and outside of the department, analyzing data/information from multiple sources.

Requirements

• Graduate of an accredited physical therapy school.
• Valid Physical Therapist (PT) license and Basic Life Support (BLS) Healthcare Provider certification.
• Valid Driver’s License (Class C or D) and automobile insurance (required for Home Health department).
• 5 years of recent relevant experience.
• Proficiency with computers and required technology.
• Must be able to work in a demanding environment, organize multiple priorities, and complete work duties within expected timelines.

Schedule

• Full-time, non-exempt position.
• 10-hour shifts, Monday through Friday (variable schedule).
• Weekend requirements: once a month.
• Weekly hours: 40.

Pay

Pay range is $67.43 to $94.40 per hour. The compensation range may vary based on geographic location, candidate experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is one component of Sutter Health’s comprehensive total rewards program.
