Physical Therapist, Home Health

Responsibilities

• Plan, administer, and adjust skilled physical therapy treatments to improve patients' physical function.
• Assess patients' strengths, develop treatment programs, and collaborate with physicians and clinical teams.
• Supervise and evaluate Physical Therapy Assistants and Home Health Aides, ensure documentation accuracy.
• Document patient care, progress, and communicate updates to physicians, patients, and families.

Qualifications & Skills

• Degree from an accredited Physical Therapy Program (APTA approved).
• Current, unrestricted Physical Therapy license and CPR certification.
• Preferred: at least one year of experience.
• Strong organizational and communication skills.
