About the role
We are seeking a Home Health Physical Therapist to work one-on-one with patients in their homes, helping them regain strength and independence in a compassionate, supportive culture.
Responsibilities
- Provide personalized therapy to patients in their homes
- Assess patient needs and apply generally accepted therapy standards
- Demonstrate good interpersonal skills and knowledge of durable medical equipment
Requirements
- Bachelor of Science in Physical Therapy
- Valid State License in Physical Therapy
- Current CPR certification
- At least two years' experience in acute care or rehabilitation settings
