About the role

Deaconess Home Health in Jasper, IN is hiring a part‑time Physical Therapist. The role provides physical therapy services to patients in their homes, promoting rehabilitation, recovery, and improved quality of life. You will work as part of an interdisciplinary team, collaborating with patients, families, caregivers, and other healthcare professionals to develop and implement personalized treatment plans that address each patient’s unique needs and goals. The position utilizes NestMed ambient AI technology.

Responsibilities

• Conduct evaluations of patients’ physical abilities, mobility, strength, and functional limitations.
• Implement therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function and mobility.
• Educate patients and their families about exercises, safety, and lifestyle modifications to promote recovery and prevent re‑injury.
• Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Maintain accurate and up‑to‑date patient records in compliance with state regulations and agency policies.

Requirements

• Bachelor’s degree from an accredited school approved by the American Physical Therapy Association.
• Active registered PT license in the state where you practice.
• Current CPR/BLS certification.
• One‑year physical therapy experience in a teaching or clinical setting is preferred but not required.
• Current driver’s license.
• Comply with federal, state, and local laws and regulations.

Benefits

• Mileage reimbursement at 58.5 cents per mile.
• No after‑hour on‑call requirements.

Schedule

Part‑time, 3 days per week, 8‑hour day shifts (8:00 AM – 5:00 PM).

Pay

Pay range: $39.18 – $58.28 per hour.
