PHYSICAL THERAPIST - HOME HEALTH

Responsibilities

• Provides direct and indirect patient care to assist in the rehabilitation and restoration of normal body function after illness and/or injury.
• Implements treatment programs according to the plan of care, administers modalities, trains patients in T.E., transfers, and gait, and supervises PT Assistants and PT Aides.
• Physical Therapy Assessment and Skilled Treatments.
• Client Care Coordination.
• Teaching, Supervision and Counseling Clients and Caregivers.
• Documentation of Interdisciplinary Communication.
• Supervision of Ancillary staff.
• Problem Solving and Critical Thinking Skills.
• Documentation.
• Able to effectively and safely transfer patients with significant physical and/or mental impairment.
• Able to manage time effectively to develop a plan of care.

Qualifications

• Ability to assist in patient lifts, transfers, and guarded functional activity training.
• Minimum required: Bachelor's degree.
• Preferred/desired: Master's degree or doctorate.
• Essential: Competencies including clear communication skills, both written and verbal, ability to keep confidential information regarding patients, team members, and customers, skills to provide customer service, ability to withstand crisis situations, and skills to provide customer service to patients, team members, and visitors.
• Knowledge and experience with electronic health records, Excel, PowerPoint, Word, Visio, etc.
• Valid DL with no more than 3 moving violations.
• Credentials: PT - Physical Therapist, BLS - Basic Life Saving certification.

Pay

Entry USD $38.81/Hr. Max USD $60.16/Hr.
