Physical Therapist, Home Health

Responsibilities

• Plan and deliver personalized physical therapy treatments to improve patients' strength, mobility, and independence.
• Assess patients' physical capabilities to support treatment planning and set measurable objectives.
• Develop and adjust rehabilitation programs in collaboration with physicians and the care team.
• Supervise and evaluate Physical Therapy Assistants and Home Health Aides, ensuring proper documentation and adherence to care plans.
• Document patient progress, interventions, and evaluations accurately and promptly, communicating with physicians and team members.
• Report patient progress and submit necessary documentation such as treatment plans and discharge summaries.
• Administer treatments, collaborate with the medical team, supervise staff, document care, and communicate patient progress to optimize recovery and functional outcomes.

Qualifications & Skills

• Degree from an accredited Physical Therapy Program (APTA approved).
• Current and unrestricted Physical Therapy license.
• CPR certification.
• Strong organizational and communication skills.
• Valid driver’s license, auto insurance, and reliable transportation.
• Preferred: minimum one year of experience.
