Responsibilities

• Provide geriatric care services in patients’ homes and senior living communities.
• Collaborate with interdisciplinary teams to develop and implement treatment plans.
• Assess patient needs and modify treatment plans accordingly.
• Document patient progress and communicate with healthcare providers.

Requirements

• Bachelor’s degree in Physical Therapy or Occupational Therapy.
• Current license to practice in at least one state.
• Experience in geriatric care preferred.
• Excellent communication and interpersonal skills.

Qualifications

• Ability to work independently and manage time effectively.
• Strong problem-solving and critical thinking skills.
• Proficient in Microsoft Office Suite.
