Responsibilities

• Conduct comprehensive assessments to design innovative, evidence-based treatment plans tailored to each individual's lifestyle and goals
• Implement high-impact therapeutic interventions with compassion, guiding patients through rehabilitation
• Meticulously monitor and document clinical milestones, celebrating progress toward full empowerment
• Collaborate with a multidisciplinary team of healthcare experts to ensure holistic, seamless, and superior care

Requirements

• Valid and active Physical Therapist license in the state of Michigan
• Experience in physical therapy, especially in a home health setting (preferred but not required)
• Strong communication skills and a friendly demeanor that connects with patients and their families
• Ability to work independently and manage time effectively
• Compassionate approach to patient care and a desire to learn and grow in your role
• Valid driver's license and reliable transportation for home visits
