Description
The Rehabilitation Services Physical Therapist provides evaluation and treatment of patients in a range of specialty programs.
Requirements
- Bachelor’s Degree or higher in Physical Therapy from accredited university
- Licensed as a Physical Therapist in the State of Michigan
- CPR Certification
Responsibilities
- Maintains good rapport and cooperative relationships; approaches conflict in a constructive manner
- Identifies problems, offers solutions, and participates in their resolution
- Maintains confidentiality of patient, physician, colleague, and visitor information
- Adheres to the Code of Conduct, Service Excellence Standards, and the Mission, Vision, Values and policies of Chelsea Hospital
- Performs job duties safely and reports hazards immediately to management
- Demonstrates self-directed learning and provides quality patient care considering age-specific, developmental, and cultural needs
- Demonstrates unit/area competencies and assists in resolving safety, sanitation, and maintenance issues
- Practices effective communication in all interactions (patient, visitor, co-worker, etc.) and promotes a positive work environment through teamwork
- Conducts individualized assessments using a variety of measures and procedures
- Provides thorough physical therapy evaluation and creates an appropriate plan of care based on patient deficits
- Stays up to date on participating insurance guidelines for rehabilitation services
- Provides thorough, timely documentation compliant with state law, accrediting agencies, and third-party payers
- Formulates individualized, age-specific therapy goals based on assessment results and patient needs
- Consults with team members to select the most appropriate plan of care
- Implements patient care consistent with therapy goals and changing patient needs using a variety of therapeutic approaches
- Completes daily charges for each patient while adhering to ethical practices per insurance guidelines
- Orients patients and families to therapeutic purposes and educates them about home programs, impairments, and progress
- Provides daily oversight of physical therapy provided by staff (e.g., physical therapist assistant, athletic trainer, or physical therapy aide) and co-signs documentation when required
- Performs regular re-evaluations/re-assessments to ensure progression toward goals
- Supervises Physical Therapy student affiliates as assigned by the Manager or Student Program Coordinator
- Pursues professional growth and contributes to staff development activities
- Works with Manager and secretarial staff to manage schedules for targeted productivity and clinic budget
- Participates in interdisciplinary team meetings 1-2 times per week (required for Acute Care physical therapists on the IP Rehab Unit)
- Attends regularly scheduled staff meetings and in-services
- Performs other duties as assigned
