Physical Therapist - Full Time

Responsibilities

• Assess patients' therapeutic and functional status
• Develop and implement therapy plans aligned with physician orders
• Guide patients and caregivers in supportive techniques, safe mobility, and education
• Evaluate the home environment
• Instruct family members
• Coordinate with the interdisciplinary team
• Document patient progress
• Communicate with healthcare providers
• Supervise Physical Therapy Assistants
• Participate in care planning

Qualifications

• Bachelor’s or master’s degree in physical therapy
• Licensure
• CPR certification
• Strong organizational skills
• PREFERRED: Experience in home health or community-based therapy
• PREFERRED: Valid driver’s license for travel
