Position Duties
- Perform therapy treatments and functions for skilled nursing patients.
- Create and implement treatment plans to help patients reach their maximum potential or successfully transition to the next level of care.
- Collaborate with other therapists and nursing staff to ensure cohesive quality of care.
- Educate patients and caregivers on how to succeed at home after discharge from the facility.
- Provide quality documentation as required.
