About the Role

Provide patient-centered care in a home health setting to improve mobility and independence.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Administer skilled care based on physician’s orders and care plans.
• Educate patients, families, and healthcare team members.
• Participate in specialty programs such as Balance and Fall Prevention, Spine Safety, and joint replacement recovery.
• Deliver intermittent professional services and support patient mobility and daily living skills.
• Follow care protocols under supervision.

Requirements

• Valid state driver’s license and automobile liability insurance.
• Dependable, well-maintained transportation.
• Ability to drive in various weather conditions.
• Current state licensure as a Physical Therapist.
