We recently opened our new 100,000 square foot facility that houses our Family Practice Clinic, Wellness and Rehabilitation, and Retail Pharmacy. About the role
As a Physical Therapist at Haxtun Health, you'll be a vital part of our team, acting as a "movement detective" to evaluate and pinpoint the root causes of patients' pain and limitations. You will develop personalized rehabilitation programs to enhance mobility, alleviate pain, and restore function, directly witnessing the positive impact of your hands-on expertise. Beyond clinical care, you'll have opportunities to nurture community wellness through local outreach and preventative programs while building strong, supportive relationships with both patients and colleagues.
Responsibilities
- Evaluate and diagnose movement dysfunctions and pain sources in patients.
- Develop and implement personalized rehabilitation programs to improve mobility, reduce pain, and restore function.
- Engage in community wellness initiatives and preventive care programs.
- Collaborate with colleagues to provide high-quality, compassionate care.
Qualifications
Minimum Qualifications:
- Graduate of an accredited program in physical therapy (MSPT or DPT).
- Current license as a Physical Therapist in the State of Colorado.
- Basic Life Support (BLS) certification.
Preferred Qualifications:
- Three years’ experience in inpatient and outpatient settings.
- Trigger Point Dry Needling Certification.

- Paid Time Off (PTO): Accrued at approximately 17 days for the first year of employment for hourly employees and 22 days for the first year of employment for salaried employees. PTO accrual increases as your length of service increases. This includes holidays.
- Sick Leave: Up to 48 hours per year.
- Retirement Plan: Up to a 3% employer match, which increases with your length of service. New employees are automatically enrolled at a 1% contribution rate (can be opted out). Employees must complete 5 years of employment to be 100% vested.
- Employee Health Payment Match: Haxtun Health matches employee payments up to $1,000 per year per family for services received at Haxtun Health’s hospital or clinics.
- Family and Medical Leave Insurance Program (FAMLI): Haxtun Health participates in the CO FAMLI program, which provides up to 12 weeks of paid leave for qualifying life events.
- Term Life Insurance: 1 x annual salary term life provided by HHD, no employee premium ($200,000 Max). Additional voluntary life insurance coverage for the employee and family is an option which can be purchased by the employee.

Pay
Starting Pay Range: $100,880.00/yr - $131,144.00/yr, depending on experience.

Full-time, exempt. Day shift.
