Physical Therapist, FT and PRN

About the Role

We are seeking a highly skilled and motivated Physical Therapist to provide physical therapy services to our patients, promoting excellence in rehab care, interdisciplinary cooperation, and quality patient outcomes. We offer a dynamic and supportive work environment.

Responsibilities

• Evaluate patients within timely fashion, as required by state, payor source, or facility requirements.
• Provide physical therapy services to patients, including evaluation, treatment planning, and implementation.
• Communicate with other disciplines and interact with staff as necessary, as indicated by the treatment plan.
• Provide routine documentation according to Medicare, facility, and other payor source guidelines.
• Participate in family conferences when requested.
• Record and submit in timely fashion all required documentation and treatment charges, according to policy and procedure.

Requirements

• Current Massachusetts State License as a Physical Therapist.
• Flexibility to be part of a dynamic and cooperative team, distributing workload equitably.
• Capability to evaluate or assess patients, participate in consultations, and collaborate with residents, staff, family, and other professionals to plan individualized treatment plans and goals.
• Ability to discuss and plan appropriate discharge plans, and teach and counsel patients and staff.
