About the role

Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services for hospice patients in a home setting. Join our team and help older adults maintain their independence and quality of life through compassionate, evidence-based care.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting
• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
• Active GA Physical Therapist (PT) license
• New graduates and experienced therapists are encouraged to apply
