Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services for Hospice Patients in a home setting. Join our team and help older adults maintain their independence and quality of life through compassionate, evidence-based care.

This PRN position offers a flexible schedule, allowing you to choose assignments that fit your availability. Treat patients in one convenient location with no driving between homes, and enjoy clinical autonomy within a supportive team environment.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting
• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR

Requirements

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
• Active Florida Physical Therapist (PT) license

New graduates and experienced therapists are encouraged to apply.
