The role involves developing personalized care plans, delivering telehealth evaluations and treatments, and utilizing digital platforms to monitor patient progress. Act as a liaison between patients and in-clinic providers to ensure seamless care delivery, supporting program growth and operational needs. Maintain accurate documentation, analyze service metrics, and participate in program development and marketing efforts. Qualifications & Skills Diploma from a CAPTE-approved Physical Therapy ProgramFlorida PT license or in process, with willingness to obtain licensure in other statesMinimum 2 years outpatient clinical experienceBilingual in English and SpanishExperience with digital health services preferredStrong communication, documentation, and patient care skills Key Responsibilities Deliver in-person and telehealth physical therapy services to diverse patient populationsDevelop and implement individualized treatment plansUse digital tools for evaluations, remote monitoring, and outcome measurementCollaborate with clinicians and support digital service growthMaintain detailed records, analyze regional service data, and contribute to program development
