Physical Therapist Case Manager: Home Health

About the role

The ideal candidate will manage physical therapy cases in home health settings, ensuring patients receive comprehensive care plans and progress towards their goals.

Responsibilities

• Develop and implement individualized treatment plans based on patient assessments
• Communicate regularly with patients, families, and other healthcare providers to monitor progress and adjust treatment plans as necessary
• Ensure compliance with regulatory standards and insurance requirements
• Document all interactions and treatments accurately and timely
• Participate in staff meetings and training sessions

Requirements

• Bachelor's degree in Physical Therapy from an accredited program
• Current California Physical Therapy license
• At least 2 years of experience in home health or related field
• Strong communication and interpersonal skills
• Proficiency in Microsoft Office Suite
