About the role
Responsible for managing the care of patients in their homes, ensuring they receive appropriate physical therapy treatments and support.
Responsibilities
- Assess patient needs and develop individualized treatment plans
- Provide direct physical therapy services to patients in their home environment
- Collaborate with other healthcare professionals to coordinate care
- Document progress and communicate with patients, families, and referring physicians
- Ensure compliance with regulatory standards and insurance requirements
Requirements
- Bachelor's degree in Physical Therapy from an accredited program
- Current California Physical Therapy license
- At least 2 years of relevant work experience in home health or related field
- Excellent communication and interpersonal skills
- Ability to travel to various locations within the service area
