Physical Therapist Case Manager- Home Health- 0.7 FTE

ProHealth Home Care is one of Wisconsin's most comprehensive sources of medical care and hospice care provided in the home. We provide a seamless continuum of care for people of all ages. This is not a seasonal or temporary position.

Responsibilities

- Works collaboratively with patients, nurses, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care environment. - Assesses plans, facilitates and evaluates timely coordination of quality, cost-effective care. - Works flexible hours and has 24-hour accountability as necessary to meet patient/family care needs.

Requirements

- Bachelor's Degree in Physical Therapy or higher. - State of WI Physical Therapist licensure. - Valid State of WI Driver's License with acceptable driving record. - Current BLS (CPR) certification or ability to obtain upon hire. - At least 6 months experience in a home health setting preferred. - At least 1 year physical therapy experience preferred.
