• Monday – Friday
• 1 Weekend Rotation per month

Responsibilities

• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of Physical Therapy.
• Provides the appropriate level of clinical direction for Physical Therapy Assistants.
• Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient's needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
• Manages the activities of the multi-disciplinary team providing care to the patient.
• Maintains communication with all disciplines regarding the provision of therapeutic services.
• Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
• Interprets agency policy to patients/families.
• Reviews patients' visits weekly and provides a plan to the Patient Care Supervisor and team assistant, including anticipated weekend visits.
• Communicates schedule changes and patient care issues to the Patient Care Supervisor.

Requirements

• BLS (Basic Life Support)
• PT (Physical Therapist) License
• Driver's License (Florida driver's license required within 30 days of hire)

Education

• Bachelor's Degree in Physical Therapy
• Prioritizing preferred but not required

Experience

• One year of Physical Therapy experience
• One year of Home Care experience
