Physical Therapist - Birth to Three - Early Intervention

Major Responsibilities

• Home visitation with infants and toddlers and their families
• Conduct developmental screenings and/or eligibility evaluations
• Aid families in implementing functional, age-appropriate strategies
• Develop Individualized Family Service Plans (IFSP) with families
• Provide service coordination to families
• Aid children in transitioning to preschool programming
• Collaborate with a transdisciplinary team of Early Childhood professionals

Qualifications

• CT DPH Licensed/Certified Physical Therapist
• Experience with infants, toddlers, and families
• Service Coordination training (or willingness to obtain)
• Excellent organizational skills and computer literacy
• Driver’s license and auto insurance required

Physical Demands

Regular and consistent attendance is considered an essential function of this position. The Physical Therapist works under the supervision of the Birth to Three Director in the division of Early Childhood & Family Programs.
