Physical Therapist

Responsibilities

• Perform evaluations and develop individualized care plans
• Deliver evidence-based therapy
• Support patient independence and mobility
• Schedule treatments flexibly
• Incorporate telehealth
• Collaborate with a multidisciplinary team

Requirements

• A valid Physical Therapy license
• Basic computer skills
• CPR certification
• Physical ability to perform community-based care tasks

Qualifications

• Passionate about providing in-home care for older adults
• Familiarity with the Geriatric House Calls™ model
• Flexibility in scheduling
• Ability to work independently and as part of a team
