Physical Therapist

Role: Licensed Physical Therapist providing in-home and senior living care to older adults, utilizing a flexible, patient-centered approach. Responsibilities Deliver one-on-one physical therapy in private residences and senior living communities.Conduct evaluations, develop individualized care plans, and provide evidence-based treatments.Support patient wellness, mobility, independence, and safety through therapy sessions.Document care effectively within EMR systems, adhering to regulatory standards.Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists.Educate patients, caregivers, and staff to promote aging in place.Utilize telehealth and virtual care to extend access and flexibility. Qualifications & Skills Valid Physical Therapy license in applicable state(s).Degree from an accredited Physical Therapy program.Proficiency in computer and EMR use.Current CPR certification. Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, reposition patients, and meet the physical demands of community-based care, including driving and wearing PPE as needed.
