Responsibilities
- Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life.
- Document patient progress in EMR, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff on safety and aging in place.
- Offer telehealth/virtual care to enhance access and continuity.
Qualifications & Skills
- Valid PT license
- Degree from an accredited program
- Basic computer/EMR skills
- CPR certification
- Able to meet physical demands such as lifting and assisting patients
