Role Overview We are seeking a Full-Time Physical Therapist to provide skilled home health services, focusing on patient recovery, mobility, and independence. Responsibilities include evaluating patient needs, developing and adjusting personalized treatment plans, collaborating with an interdisciplinary team, educating patients and caregivers, and documenting progress accurately. The role emphasizes delivering compassionate care in patients' homes to improve their quality of life.
