Physical Therapist

Role: Licensed Physical Therapist providing personalized in-home and senior living care to older adults, utilizing the Geriatric House Calls™ model. Responsibilities: Conduct evaluations, develop individualized treatment plans, deliver evidence-based therapy sessions to enhance mobility and independence, document in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate. Qualifications & Skills: Valid PT license, degree from an accredited program, computer proficiency, CPR certification, and ability to meet physical demands such as lifting, repositioning, and driving.
