Physical Therapist
Duties
- Perform Start of Care (SOC), Evaluations, Resumption of Care (ROC), and Recertifications
- Provide ongoing therapy visits and reassess patient progress
- Develop and implement individualized treatment plans to improve mobility and function
- Educate patients and caregivers on therapeutic exercises and safety practices
- Maintain accurate and timely clinical documentation
- Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes
- Ensure compliance with home health regulations and agency policies
Requirements
- Education: Doctor of Physical Therapy (DPT) required
- Licenses: Active Physical Therapist (PT) license (state-specific)
- Certifications: Current CPR certification
- Experience: 1+ year

Pay
$50/hr - $56/hr *Pay offered is based on experience, expertise, credentialing, and education.

Monday-Friday 8:00 am to 4:30 pm/ 40 hrs/ week
