Licensed Physical Therapist providing personalized, in-home, and virtual care to older adults in various settings, including private residences and senior living communities.

Responsibilities

• Conduct evaluations
• Develop individualized treatment plans
• Deliver evidence-based therapy sessions to improve mobility, independence, and quality of life
• Document care in EMR
• Collaborate with interdisciplinary teams
• Educate patients and caregivers
• Incorporate telehealth when appropriate

Qualifications & Skills

• Valid Physical Therapy license
• Degree from an accredited program
• Basic EMR proficiency
• Current CPR certification
• Physical ability to perform patient transfers, mobility support, and meet physical demands of community-based care
