Physical Therapist

Role: Physical Therapist providing personalized, one-on-one care to older adults in their homes or senior living communities. Responsibilities: Conduct evaluations, develop and implement individualized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life. Document care using EMR systems, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff on safety and aging in place. Incorporate telehealth and virtual care to enhance access and flexibility. Qualifications: Valid Physical Therapy license, degree from an accredited program, basic computer skills, CPR certification, and ability to meet physical demands such as lifting and assisting patients.
